For this event there is a £50 "No Show" fee, if you do not attend or cancel 48hrs prior to the event. Once place has been confirmed, we will be contacting ALL confirmed registrants by phone to confirm attendance prior to the event.

10.00 - 10.50Management of Hypertension in Primary Care. Speaker: Jagdeep Sangha

Key points:

  • Population health approach to managing Hypertension in primary care and how to identify patients undiagnosed.
  • Adopting a whole primary care team approach in managing hypertension (from diagnosis to management) both within the GP practice (including patient themselves) and external assets (e.g. community pharmacy BP service).  This will support efficiency in primary care with ever increasing workload and ensure pharmacists are utilising their skills more effectively in maneging long term conditions.
  • Ensuring cardiovascular risk assessment and a wholistic approach drives treatment decisions in hypertension management in primary care, recognising the balance against contractual drivers

Key points:

  • The importance of Medication Review:  The importance of conducting regular medication reviews for patients, especially those at higher risk of polypharmacy (such as older adults or individuals with multiple chronic conditions). Highlight the role of pharmacists in identifying potential drug interactions, duplications, and inappropriate medications to optimize therapy and reduce adverse effects.
  • Patient Education and Empowerment: The significance of patient education in polypharmacy management.  Pharmacists can engage patients in discussions about their medications, including their purpose, potential side effects, and proper administration. Empowering patients to actively participate in their treatment decisions.
  • Collaborative Care Approach: Advocate for a collaborative approach to polypharmacy management involving pharmacists, prescribers, and other healthcare professionals. The utilisation of IT, clinical systems to not only support patients in decision making but also to proactively find patients who maybe at risk of harm and identify for review.
