Closing the Gap in Cholesterol Management - From Evidence to Implementation in Primary care
Wednesday 3rd June, 1pm - 2pm
In today’s rapidly evolving healthcare landscape, shaped by new models of care and the 10-Year Health Plan, pharmacists play a vital role in cardiovascular prevention and management, supporting patients with risk factors such as raised cholesterol. As key members of multidisciplinary teams, they bring expertise in operational delivery, medicines optimisation, adherence support, and lifestyle counselling.
This session brings primary care colleagues to showcase how pharmacists, working within multidisciplinary teams, can embed Lipid management and deliver sustainable population health through coordinated care.
