Integrated prescribing: Pharmacy’s role in

population health and neighbourhood care

Thursday 18th December

1pm - 2pm

Three key takeaways

  • Understand integrated prescribing models and how pharmacists contribute as independent prescribers within multidisciplinary teams to support joined-up care across neighbourhoods.
  • Recognise pharmacy’s impact on population health through proactive management of long-term conditions, preventative care, and data-driven approaches to addressing health inequalities.
  • Explore how pharmacy integrates into neighbourhood care teams, enhancing access, supporting shared decision-making, and reducing pressure on other healthcare services.
