Following the recent publication of the Cardiovascular Disease Modern Service Framework (CVDMSF) the Heart Failure Academy are delighted to present Heart Failure and the older adult: an honest conversation.

This webinar will provide a pragmatic focus on how we can implement personalised care as per the CVDMSF for older adults living with heart failure.

Learning outcomes

By the end of this webinar viewers will be able to:

  • Describe the importance of community services in the care of older adults living with heart failure
  • Give examples of the complexities of supporting an older adult living with heart failure considering:
  1. health inequalities
  2. frailty
  3. co-morbidities
  • Discuss the practicalities of:
  1. NTproBNP testing in older adults
  2. the limitation of the evidence base in heart failure guideline directed medical therapy in older adults
  3. person centred care for older adults living with heart failure

Pre-webinar

This presentation will build on a basic understanding of NICE NG 106 Chronic heart failure in adults: diagnosis and management (updated 2025).

Shannon has led person-centred care for older people in primary, secondary care and postgraduate education for both pharmacy professionals and the wider multidisciplinary team.

Shannon is currently pathways lead for the primary care pharmacy education (Midlands and East) and is education lead for the Heart Failure Academy which aims to upskill the generalist multidisciplinary team to support heart failure care in Yorkshire and the Humber.

Dr Fiona Carr

Consultant Geriatrician with a specialist interest in heart failure, Sheffield Teaching Hospitals NHS Trust

Dr Carr has a unique role as a consultant geriatrician with a specialist interest in heart failure and cardiothoracic medicine, employed by Cardiology at Sheffield Teaching Hospitals NHS Trust. Dr Carr sees older adults living with heart failure on the wards and in outpatients as well as being on the trust’s Advance Care Planning Panel.
